Pressure relief is often the selected basis of safety for process plant items.
It is typically the last line of defence where its operation is required after everything else has failed.
The correct sizing and installation of the pressure relief device forms part of the users risk assessment in the prevention or mitigation against the consequences of process plant upset or emergency conditions.
However, pressure relief provision can and often does goes wrong, with the user potentially unaware of the inadequacies of the design and / or installation until it is too late.
